Grenfell Community Case Management is an NHS service specifically for Grenfell-affected patients living in North Kensington aged 18-64. The service provides case management and care co-ordination for adults with complex health needs, in partnership with GPs and other community-based services.

Adults over the age of 65 are managed within the exisiting My Care My Way service that is available to all patients in Kensington & Chelsea.
