Criteria to becoming a health partner

• Be a registered charity, voluntary or community group, Social Enterprise or Community Interest Company or registered social landlord.
• Disclosure and Barring Service (DBS) check or advanced DBS as appropriate.
• Have a completed Data Security and Protection (DSP) Tool Kit or be willing to complete this prior to the commencement of the service.
• Not have received funding from another source for the same service (however, an organisation may apply again, for a new service, or an adapted service informed by lessons learned).
